Counseling Agreement and Informed Consent

This is a ministry providing Biblical counseling. We offer counseling to individuals seeking to integrate their faith into the process of resolving personal, spiritual, or family issues. Our counseling is not licensed by the state.

We counsel each person based on her unique personality, experiences, and circumstances. We believe that the secret of a happy and healthy life is to attempt to maintain a balanced lifestyle. This can be achieved through proper planning with guidance from the Bible, the Holy Spirit, Christian friends, Biblical counseling, and knowledgeable professionals.

Many problems are caused by dysfunctional or false beliefs that have developed over time, and these belief systems take time to correct. The process of “renewing your mind” takes time, so be patient and do not set unrealistic expectations for yourself.

In secular psychology, there are approximately 200 different, and sometimes conflicting, philosophies. Our approach is from a Christian perspective. We base our counseling on the Bible and encourage Biblical solutions. Christian resources, such as recordings involving the Bible, books by Neil Anderson, John Reiger, James Friesian, Ed Smith, James Wilder, Dallas Willard, and other authors will be suggested upon request.

The counselor is a consultant only, whose intervention may be freely accepted or rejected by the client at any time. Therefore, decisions made during and after counseling sessions are the responsibility of the client. Even though you can stop counseling sessions at any time, if you have an appointment, please try to be on time or cancel at least three hours prior to the appointment.

There are times when clients feel the need for a “father,” and hope that the counselor can meet this need. They may also have tactile needs (to be touched, hugged, etc.). The counselor will not try to meet these needs. We expect any person we counsel with to be honest and avoid yelling, hitting, throwing objects, or using profanity. Each person should avoid “transference” of feelings of love, affection, anger, sexual desire, etc. onto the counselor. If these standards are broken or something is done to compromise the counselor’s integrity, the client may be asked to leave, or be referred to another counselor. Please respect these standards, as they are for everyone’s benefit. During your counseling sessions, there will either be a female prayer partner present or a video monitor used. We would ask that you dress in an appropriate and modest manner.

Our main concern is your improvement, and not blaming or pursuing any alleged perpetrators. If it is discovered that you have a disorder outside the realm of our expertise, we may refer you to another professional or institution. If a person self-mutilates, or if there is an imminent threat of harm being done to oneself or others, we may encourage hospitalization. Law mandates us to report any cases of child abuse.

We have a strict confidentiality policy, and you can be assured that your privacy will be respected by all of our staff. We do encourage family participation, and with your permission, may discuss things with them as needed.

I understand that the staff at Door of Hope Biblical Counseling Ministry are not professional or licensed counselors, therapists, psychiatrists, medical or psychological practitioners, or if they are licensed in one of these areas, they are not practicing within that license.

I fully understand that the ministry I receive is not counseling in any form, but rather prayer ministry. I accept this ministry fully and completely, and do not hold anyone responsible for any outcome that may arise as a result of this ministry. I also do not hold any one responsible for any further or additional care that I may need in the future. I take full responsibility for my life, health, and well being, both now and in the days to come following my time at Door of Hope.

I am voluntarily accepting this offer of help and/or training, and acknowledge that I understand that I am free to terminate my participation at any time for any reason.

I understand that I must take full responsibility for any and all consequences for prematurely terminating any therapeutic/spiritual intervention done for or on my behalf.

I understand that once I leave Door of Hope, I accept full responsibility for any choices that I make that may be detrimental or harmful to me. I also accept full responsibility for all after-care and follow-up counseling. I recognize that Door of Hope does not provide any promise of future ministry after my time here has ended.

I understand that some or all of the procedures used to give spiritual and emotional help to me may not be clinically demonstrated as guaranteeing results, either short-term or long-term.

I understand that if at any time I display violence or injury towards myself or others, my time at Door of Hope and the Hannah House will be terminated.

I understand that I am under no financial obligation.

I give my permission for the counselor to consult other qualified professions to seek information or advice that will assist in the counseling process. I understand that in such cases every effort will be made to preserve my confidentiality.

I am aware of my right to ask for clarification of any part of this statement of understanding.

You will be contacted shortly after we receive your application.

Please note that we do require the included reference pages to be filled out by a pastor, mentor, close friend, etc. from your church/area and sent back to us before we can approve your application.

Hannah House Ministry

INTRODUCTION
The Hannah House Ministry (herein after referred to as HHM) is a short-term program for adult ladies who need to find emotional and spiritual healing.

The vision for the HHM came from hearing the deep cry from many young ladies who have been wounded, hurt, and abused, and need a place of rest and safety while working through the process of healing.

The housing arrangement is several cabins in a community style setting with House Parents and a House Mom living in one of the cabins. While staying at the HH, each lady will receive guidance and encouragement from the House Parents and House Mom.

This happens in a serene, tranquil, and secluded setting in the Foothills of the Blue Ridge Mountains of Columbus, North Carolina, surrounded by God’s creation, such as animals, grassland, trees, streams, and nature.

The length of stay is 3 weeks during which time we work towards healing and equipping to continue the journey towards wholeness as the ladies return home. Our passion is for each heart to find complete and total healing, found only through Jesus Christ.

The HH is under the umbrella of Door of Hope Biblical Counseling Ministry, a non-profit organization based in Columbus, North Carolina.

Our Mission
Opening doors in young women's lives to let Jesus bring them to a place of healing, freedom, and hope; introducing them to the treasures of the Kingdom of God and helping them to find their identity as a daughter of God.
Hannah House Vision
To provide a place of rest and safety for girls who have been wounded, abused, and broken.

To give acceptance, mentoring, and guidance to each individual during their stay at the HH.

Our desire is to give hope, freedom, and impart God's healing to the gentle hearts of His daughters who have been wounded.

Hos 2:15 And I will give her, her vineyards from thence, and the valley of Anchor for a door of hope: and she shall sing there, as in the days of her youth, and as in the day when she came up out of the land of Egypt.

HANNAH HOUSE MINISTRY PROFILE

The HHM is an intervention ministry with the following purpose and goal:

PURPOSE – We are called and anointed by God to respond to the cry for help from young women who are in need of a place to find healing, love, acceptance, worth, and purpose.

The one thing that caused Jesus to weep when He was here on earth was the multitudes of hurting people. The silent screams of today's multitudes of wounded individuals continue to reach His ears and touch His heart. His tears continue to flow for the hurting daughters of God.

Our ministry is to women who have experienced trauma and/or abuse and are in need of a place to go to find rest and healing.

“Behold, the voice of the cry of the daughter of my people..., for the hurt of the daughter of my people am I hurt: I mourn; dismay hath taken hold on me. Is there no balm in Gilead? Is there no physician there? Why then is not the health of the daughter of my people recovered?” Jeremiah 8:22

GOAL – HHM is committed to connecting the hearts of individuals to the heart of their Creator by finding healing for wounded emotions. This is done by working through spiritual strongholds, lie-based thought patterns, and iniquities in order to experience freedom in their heart to be who God created them to be.

The HHM is under the umbrella of Door of Hope Biblical Counseling Ministry, a non-profit organization based in Columbus, North Carolina, and does not receive women mandated by state agencies or the accompanying funds associated with such. HHM operates within the parameters of its stated purpose and it's staffing potential, and relies solely on the contributions of clients and other interested parties.

HHM reserves the right to refuse acceptance of any applicant into the program that would place the applicant or other residents at undue risk.

The following conditions are examples but not limited to those that may be denied admittance into HHM residential program:
